About the Role

We are seeking a dedicated and experienced Social Worker to join a hospital-based multidisciplinary team in regional Queensland within a rural generalist setting.

This is a varied and rewarding role where you will support patients across the lifespan, with a strong focus on discharge planning, patient flow, and ensuring safe and effective transitions between hospital and community care.

Working closely with nursing, medical, and allied health teams, you will provide holistic psychosocial assessments, brief interventions, crisis support, and care coordination for patients and families experiencing complex health and social needs.

As the primary social work resource on site, you will have a high degree of autonomy while being supported by a collaborative and experienced clinical team and broader regional network.

Key responsibilities include:

  • Hospital-based social work across acute and sub-acute settings
  • Leading discharge planning and supporting patient flow
  • Conducting psychosocial assessments and intervention planning
  • Supporting patients and families across the lifespan
  • Liaising with community services and external supports
  • Contributing to multidisciplinary case conferences and care planning
  • Working independently while being part of a supportive team environment

About You

We are looking for a confident, compassionate Social Worker who thrives in a rural generalist hospital environment and enjoys working across a broad and diverse caseload.

You will bring:

  • A recognised qualification in Social Work
  • Eligibility for membership with the Australian Association of Social Workers (AASW)
  • Experience in hospital social work and/or rural generalist practice
  • Strong background in discharge planning and care coordination
  • Excellent communication and stakeholder engagement skills
  • Ability to work autonomously and manage competing priorities
  • A collaborative, patient-centred approach to care

Experience working in rural or regional health settings will be highly regarded.
